| Sales Associate or Broker Sales Associate – Become Active | This application is used when a licensee wants to activate their license with a real estate company. |
| Add/Remove PA, LLC, PL, or PLLC – Sales Associate or Broker Sales Associate (Form RE 16) | This form is used to add the PA, LLC, PL, or PLLC designation to the legal, licensed names of sales associates or broker associates. |
| Become Inactive – Sales Associate or Broker Sales Associate (Form RE 11) | Use this form to deactivate a license status under your qualifying broker. |
| Downgrade Broker License to Sales Associate License (Form RE 15) | This application is used when a broker fails to complete post-licensing education by their first renewal. It downgrades the null and void Broker's license to a sales license. |
| Name Change – Sales Associate or Broker Sales Associate (Form RE 10) | Use this form to change your name as a sales associate or broker sales associate. |
| Personal Address Change For Sales Associate or Broker Sales Associate (Form RE 10) | If you have recently moved, notify the Commission of your new address change using this form. To utilize this form online, you must have a license linked to your online account. |
| Add/Terminate Sales Associate(s) or Broker Sales Associate(s) (Form RE 11) | This form is used when a broker wants to activate or deactivate a sales associate's status with the broker. |
| Sales Associate Initial Application - Mutual Recognition Application For Non-FL Residents Only (RE 1) | Use this form to apply for your real estate reciprocity license in Florida. Only use this form if you're a non-resident sales associate with a license equivalent to a Florida Sales Associate license from one of the ten states with a mutual agreement with Florida. |
| Broker Initial Application - Out of State Experience (RE 2) | Use this form to apply for your real estate reciprocity license in Florida. Only use this form if you're a non-resident sales associate with a license equivalent to a Florida Sales Associate license from one of the ten states with a mutual agreement with Florida. |
| Initial Application-Broker License-Upgrade From Sales Associate to Broker (Form RE 2) | Use this form to upgrade from a Sales Associate license to a Broker license. |
| Application-Temporary Qualifying Broker Registration (Form RE 17) | If the sole broker of your brokerage dies or is unexpectedly unable to remain a broker, use this form to register on a 60-day emergency basis as a temporary qualifying broker. |
| Request For Multiple License-Broker License (Form RE 13) | This transaction is used when a broker wants to qualify more than one sole proprietorship or real estate company. |
| Real Estate Broker – Military Endorsement | This form is for people who are currently or were once active-duty members of the U.S. Armed Forces, or spouses or surviving spouses of such members. If you meet the requirements, you won't need to take a Florida exam. |
| Education Dollar For Duty Program | The Florida Department of Military Affairs may reimburse National Guard members for licensing and certification examination fees and CE costs as a part of the Educational Dollars for Duty program. Learn more about your eligibility requirements here. |
| Application For Low Income Waiver of Initial Licensing Fee | If you are low-income, the Department can waive your initial real estate licensing fee. To qualify for this fee waiver, you must demonstrate that your annual household income, before taxes, is at or below 130% of the federal poverty guidelines. |
| Military Member/Veteran/Spouse Fee Waiver and Military Service Verification Form | Complete this form if you or your spouse have served or are currently serving in the military. If you qualify, your real estate fees will be exempted. |
